What Will the Job Assignments be Like?

Variety of jobs and opportunities

Different lengths—day, week, month or longer

You are employed by BarryStaff and work on-site at our clients’ locations

Your Staff Consultant will give you details regarding each assignment.  For example-

  • Company name and address
  • Directions
  • Date and time to report to work
  • Name of person to report to and department
  • Type and length of the job
  • Skills required and special instructions
  • Any safety equipment required
  • Who to contact at client for OSHA safety training if necessary

Safety Checklist

Do’s

  • Ask Questions.
  • Lift with your legs, not with your back
  • Use dollies or hand trucks to help you lift
  • Use special care if you are working with heavy machinery
  • Know your physical limitations
  • Use safety equipment—shoes, safety glasses, etc.
  • Wear proper work clothes
  • Long hair should be tied back
  • Jewelry should be kept to a minimum
  • Make certain you are aware of safety procedures and follow safety rules

Don’t

  • Lift over 50 lbs.
  • Use alcohol or drugs
  • Reach into any machinery
  • Strain yourself lifting.  Get help.
  • Work more than 10 feet above floor level

What is Expected of Me on the Job?

  • You are not under any obligation to accept a job, however, when you do accept, we expect you to be there everyday and finish the job.
  • Be flexible and willing to learn new things
  • Be dependable
  • Be on time everyday
  • Have a good attitude
  • Make a good first impression—arrive early the first day
  • Do a good job
  • Follow client’s policies for breaks, smoking, safety, etc.
  • Hold personal phone calls for lunch and after work
  • Take extra care with client’s equipment and property
  • Don’t discuss your pay with client or other employees

When Should I Contact the BARRYSTAFF Office?

  • Call immediately if you cannot be at your assignment for any reason, or if you are running late.
  • If the job differs greatly from what you were told.
  • If client has not provided you with OSHA safety training
  • When you finish a job, or if your assignment is extended.
  • If the client offers you a full time position with them.
  • If you are injured on a job, call BarryStaff immediately (see more detail later in the booklet)
  • To report a change of name, address or phone number If you have any questions or concerns

Drug & Alcohol Policy

BarryStaff policy prohibits our employees from being under the influence of drugs or alcohol while on assignment at a client’s facility. In addition, BarryStaff prohibits the use, selling or possession of alcoholic beverages or illegal drugs while on an assignment.

How Do I Get Paid?

  • You are paid by BarryStaff.
  • We take only the deductions required by law, including taxes and social security
  • You are paid an hourly rate for all hours worked
  • Client is charged a rate to cover your salary and our services
  • Time sheet must be in our office by Monday noon (unless it is a holiday). This is your responsibility
  • Ask a BarryStaff consultant when and how you get your check Be sure you have your timesheet filled in correctly.
  • Your pay will be on our paycard every Friday by 12:00 noon.
  • Any paper checks remaining after the office closes on Friday will be mailed to your home address.

In Case of an Accident

  • If you have a work related injury, you must Inform your supervisor & complete an accident report
  • Seek medical treatment if necessary
  • Submit to a post accident drug screen within 24 hours of the time of the accident
  • Contact the BarryStaff Office within 24 hours

In case of an accident involving a personal injury, regardless of how serious, an employee should notify the BarryStaff office as soon as possible.

If you are injured on the job, we recommend you go to Concentra Medical Centers at 228 Troy Street. The phone number there is (937) 228-8132.

Our Return to Work Policy

If you are unable to return to work following an injury, you must call the BarryStaff office during your absence and report your progress to the appropriate BarryStaff representative.

You are expected to return to work as soon as possible after your injury. BarryStaff will work closely with you and/or your physician to find suitable employment for you during the recovery transition.

We Take You & Your Work Seriously

We want you, our valued employee, to understand that we want your experience with BarryStaff to be a positive one. Our clients call us because they already have a problem or situation that needs attention. If our employees become a constant source of conflict and irritation, we will lose the client and you will lose a possible source of income. Help us to help you. Be dependable, work hard and keep in touch with the BarryStaff office.

I Understand That:

As a BarryStaff employee, I will be expected to accept appropriate work assignments whenever I am needed and listed as available, and that my work habits, attitude, personal hygiene and appearance will compliment the high standards of BarryStaff and our reputation for quality service.

If at any time I am unable to report to a job assignment (or must be late) I will call the BarryStaff office two hours prior to the start time of my shift. If I encounter problems of any kind on the job, I will call the BarryStaff office immediately and NEVER WALK OFF THE JOB UNTIL THE SITUATION HAS BEEN CLEARED WITH BARRYSTAFF.

As a BarryStaff employee, I must not ask for permanent work at the company where I am assigned, but I am free to accept a permanent job offer that has been cleared with the BarryStaff office.

As a BarryStaff employee, I am expected to treat each individual assignment with the same seriousness that is due a permanent position. I understand that the clients of BarryStaff will be depending upon me to do a first rate, professional job and to help them see that business continues as usual in whatever position I may be working.

In a situation of “NO CALL/NO SHOW” (where I have failed to report to work and failed to call before my shift started), or if I walk off the job without first contacting BarryStaff and clearing the situation with BarryStaff, or if I have avoidable conflicts that have not been cleared with BarryStaff and the client prior to acceptance of a position, it will be assumed that I have voluntarily quit my position, and under NO circumstances will I ever be permitted to work for BarryStaff in the future.

Employee Referrals:

If you have friends who are looking for work, send them to BarryStaff! Make sure they tell us you referred them. When your referral works after two full weeks (80 hours) and gets paid, you will receive $20.00 as long as you and your friend are both in good standing with BarryStaff.

A Few Words About Fraud

WARNING Any person who obtains compensation from BWC or self-insuring employers by:

  • Knowingly misrepresenting or concealing facts
  • Making false statements
  • Accepting compensation to which he/she is not entitled

Is subject to felony criminal prosecution for fraud.

Vacation Pay:

Vacation pay is available to all Employees who work a minimum of 1,500 hours during the 52 weeks preceding their anniversary date. The Employee must have worked in each quarter to be eligible. Vacation pay will be paid after their anniversary date as long as they are still working for BarryStaff.

Vacation Benefits Include:

  • 40 hours of paid vacation for the first 1,500 hours you work, and an additional 20 hours of paid vacation if you work a total of 2000 hours in a given anniversary year.
  • Hourly rate for vacation pay is the average base rate of pay you receive during the accumulation period.
  • Accumulation of hours for vacation pay begins on your employment anniversary date and is reset each year on your anniversary date.
  • You must request your vacation pay when you feel you are eligible.

NOTE: Payrolling Employees are not eligible. Vacation pay may vary per specific client contract

Holiday Pay:

Holiday pay is available to all Employees who work at least 500 hours in the preceding 16 weeks prior to a designated holiday and work a minimum of 20 hours during the holiday week. You must also work the scheduled day before and after the holiday to qualify.

NOTE: Payrolling Employees are not eligible.

Holiday Benefits Include:

  • 8 hours of pay for the following:
  • Memorial Day, Fourth of July, Labor Day, Thanksgiving Day, Christmas Day, New Year’s Day

Post-Accident Drug and Alcohol Testing Policy Acknowledgment, Test Consent and Release

I understand that if I have a work-related accident which requires outside medical treatment, I will be requested to undergo drug and/or alcohol testing within 24 hours immediately following the accident at a testing facility pre-approved by BarryStaff.

I understand that if the results of the test indicate the presence of drugs or alcohol, or if the test is not completed within 24 hours at a pre-approved facility, the following will occur:

  1. I will be ineligible for continued employment and will be considered to have been terminated for misconduct and violation of company policy.
  2. I may be ineligible for workers’ compensation benefits and unemployment benefits.

I further understand that if I refuse to submit to the test following a work related accident, I will be considered to have voluntarily terminated my employment and that if I interfere with the administration of the test I will be terminated for misconduct and violation of company policy.

I understand that I have the right to have the same sample retested by a laboratory approved by my employer, at my own expense, within 48 hours of my receipt of the test results. I, (“employee”), voluntarily give my consent to BarryStaff (“employer”) and its laboratory to perform a drug and/or alcohol urine or blood test. I authorize release of the test results to my employer, its clients, my employer’s workers’ compensation insurer, and the state unemployment office. I specifically consent to the use of the test results in any administrative or legal proceedings associated with my employment.

An exception to this policy will be made for the use of legally prescribed or over-the-counter medications. It is the employee’s responsibility to advise employer and/or the testing laboratory of the medications employee is taking or has taken. This information will be kept confidential.

I release and hold harmless employer, its employees, agents, shareholders, officers, directors, affiliates and its laboratory from any claim, demand or cause of action I may have now or in the future, against any or all of them which results from my submission to or my refusal to submit to a drug and/or alcohol test.

Employer reserves the right to interpret, change, suspend, cancel or dispute, with or without notice, all or any part of this policy. Nothing contained in the policy alters the at-will status of employment, or is intended or should be construed as a contract, express or implied.

Bus Pass/Token Authorization

I understand that if need to purchase either a weekly bus pass or a bag of bus tokens from BarryStaff, the following amounts will be automatically deducted from my paycheck:

$19.50 for a weekly bus pass
$15.00 for a bag of tokens

Walk Off & No Show / No Call Policy

I understand that if I walk off a job (leave work early without supervisor/BarryStaff permission) or don’t show up for work and do not call BarryStaff with proper notice any hours up to 40 for the week will be paid at federal minimum wage.

Statement of ADA Compliance

BarryStaff is in compliance with the Americans with Disabilities Act (ADA) which became effective on July 26, 1992 for employers with 25 or more employees.

We have reviewed our hiring policies and procedures, including application forms, employment policies and benefit plans. To the extent modifications were necessary for compliance, they have been accomplished. Furthermore, through a series of memorandums and training seminars, we have educated our employees about the ADA and will continue to do so on an ongoing basis.

We trust our customer, recognizing their corresponding obligations under the ADA, will assist us in providing employment opportunities to the disabled.

General Safety Orientation

1. Hazard Communication

Under OSHA Hazard Communications Regulations, every employee must:

  1. Be informed of all hazardous substances he/she may be exposed to in the workplace.
  2. Have access to a list of all hazardous substances in the workplace, the Material Safety Data Sheet (MSDS) prepared for each, and the written program to inform and train employees in safe handling.
  3. Be trained in the identification, labeling and handling of hazardous substances to which he/she may be exposed in the workplace.

This occupational hazard communication responsibility rests with the employer in control of each workplace. If you have questions relative to hazardous substances on your assignment or wish to be informed relative to their program, please ask your supervisor on the worksite.

2. General Safe Practices

  1. I understand the Hazard Communications Procedures (above)
  2. I understand and will comply with the General Safety rules attached.
  3. I will comply with any additional worksite safety rules or safety in effect.
  4. I will use any personal protective equipment required by my assignment.
  5. I will call my BarryStaff consultant if I am asked to change assignments after arrival on the job, (you may be entitled to a higher wage on this new job.)
  6. If I am injured on the job, I will call my BarryStaff consultant immediately.
  7. I will report any unusual or hazardous job conditions to BarryStaff immediately. I understand that I may do so anonymously

Light Industrial Assignment – General Safety Rules

  1. Safe work attire is required:
    • Shoes: No open-toed shoes permitted at any time. Canvas and nylon sport-type shoes cannot be worn unless specifically authorized by your supervisor. Industrial boots with hard toes must be worn on any lifting job.
    • Clothes: Do not wear ill-fitting or baggy clothes around machinery. Loose ribbons and dangling jewelry are safety hazards and must not be worn while working.
    • Hair: Long hair must be kept controlled behind neck and off the shoulders to prevent getting caught in the machinery.
    • Jewelry: Remove finger rings before working around machinery or electrical equipment, and while handling or moving materials.
  2. Wear appropriate safety shoes if required. Always wear a hard hat in designated “hard hat” areas; wear safety glasses or goggles, safety gloves, safety belts, etc. where such items are advised or mandatory. The customer will provide these items. If not, contact your supervisor for instructions.
    • Do not operate any equipment which, in your opinion, is not in safe condition, or for which you do not have adequate training. Call BarryStaff for instructions.
    • Obey all company rules, governmental regulations, signs, markings, and instructions. Be particularly familiar with those that apply directly to you. If you don’t know, ASK.
  3. When lifting, use approved lifting techniques; i.e., bend your knees, grasp the load firmly, then raise the load, keeping your back as straight as possible. After the object has been firmly grasped, the lifting is done by straightening the legs. Never lift while the body is twisted since this puts the entire load on the muscles of one side of the body. Ask for help when necessary to lift or move any object which, because of its weight or shape is difficult for one person to handle safely. Limit your lifting to 50 pounds.
  4. Don’t horseplay. Horseplay and practical jokes frequently cause serious injury on the job.
  5. Do not distract or startle fellow workers while they are working. Unnecessary talking, shouting, or other loud noises which may take the attention of other employees away from their work is a safety hazard to you and others.
  6. Always use the right tools and equipment for the job. Use them safely and only when authorized.
  7. Be constantly alert for moving equipment and loads. Always stand clear of operating equipment.
  8. When working with lumber, remove or bend down all protruding nails.
  9. Running in the building premises of the client, or across parking lots or driveways is not permitted. Pick up objects on the floor. Report any spilled liquids on the floor immediately. Report any unsafe conditions on your worksite to BarryStaff immediately. You may report unsafe conditions anonymously if you prefer. Whenever your are involved in any accident that results in personal injury or damage to property, no matter how small, the accident must be reported to the BarryStaff office immediately. Get first aid promptly.

Time Sheets

This is a sample copy of a timesheet. Please make sure you fill out all the fields that are required. The date, time in and out, less lunch, total for the day and the week.

Do make sure the company name is on it, as well as the supervisor’s signature. And finally, make sure you sign it, print your name, and provide your social security number.
